You will begin to receive notification at the beginning of September, approximately 90-days from the start of health plan enrollment, which is December 1, 2014. This will begin an informing process that will include notification letters, choice packets, informational materials, and phone calls from people who can help with your enrollment into a managed care health plan. These notifications are described below, in the order that you will get them.
Note: If you who get notification, you must choose a health plan. If you do not choose a health plan, one will be chosen for you. Enrollment in a health plan starts December 1, 2014.
1) Letter regarding changes to your Medi-Cal coverage
Initially, you will receive your first letter in the mail approximately 90-days before the enrollment date, which explains the changes to your Medi-Cal and how it is covered. Note that this first letter will not
include the forms needed in order to make a health plan choice (that will come in your second letter). There are other ways you can choose a health plan at any time. You can enroll by phone by calling HCO at 1-800-430-4263 (TDD 1-800-430-7077) , or by going to the Health Care Options' website
to print a blank choice form to complete, sign and mail.
2) Health Plan Choice Packets
Second, if you have not yet made a health plan choice, approximately 60-days before the enrollment date you will receive a packet in the mail that includes your Health Plan Choice Packet. It is important that you review the Health Plan Choice Packet, as it will include many important items: information specific to the county you live in, the enrollment form allowing you to choose a health plan, a Health Plan Comparison Chart, a Health Information Form
*, and Health Plan Provider Directories. These items will help you compare health plans, provide you with important health information, and allow you to select a health plan.

Included in your Health Plan Choice Packet will be the Health Information Form. Filling out this form with some of your health information will allow for your health plan to best coordinate your health care. This is a self-assessment, and it is voluntary.
3) Follow-up phone call to health plan choice packets
Third, if you have not yet made a health plan choice, you will receive a phone call from Health Care Options between the time you are sent the Health Plan Choice Packet, and before you are sent the final reminder notice (see below). This is to inform you of your Medi-Cal changes and to answer any questions you may have. You may also choose a health plan during this call.
4) Final letter reminding you to choose a health plan
Fourth, if you still have not made a health plan choice, this is one of the last steps and opportunities of the enrollment process. Approximately 30-days prior to the health plan enrollment date, a letter will be sent to you to give you another chance to make a health plan choice. If you do not make a health plan choice, a health plan will be chosen for you. This letter will inform you of what health plan will be chosen for you if you do not make a choice. Remember, you still have time to choose.
5) Follow-up phone call to final letter
Fifth, if you have not yet made a health plan choice, you will receive another call from Health Care Options to help you make a health plan choice and answer any questions you may have. You may also choose a health plan during this call.
6) Letter confirming your enrollment into a health plan
If you chose a health plan, a letter will be sent to you to confirm the health plan you are enrolled in and will start coverage with on December 1, 2014.
