  • Identify the topic by noting the IPC BOX Number that most closely reflects your question and place that BOX Number in the subject line;
  • Include only questions related to the IPC BOX Number that appears in the e-mail subject line;
  • UseOTHER if there is no IPC BOX Number for your specific question; and
  • Use a SEPARATE e-mail for each IPC BOX Number for which you have questions.

Question: When shall we start using the new IPC form for TAR submission?

Answer: For dates of service on/after February 1, 2008, AND if you are submitting the TAR on/after February 1, 2008, use the new IPC form.
