LIHP Application, Authorization and Contracting Documentation
Included below are the application and the DHCS application approval letter for each County, Health Authority, or Entity participating in LIHP implementation. Also included below is a link to each local LIHP’s submitted deliverables, contract and CMS approval letter.
Each County, Health Authority, or Entity listed below submitted an application for participation in the LIHP and received an application approval letter from DHCS. Application approval does not mean that a County, Health Authority, or Entity is authorized to implement the program.
As part of the LIHP authorization and contracting processes, DHCS requested information (or “deliverables”) from each County, Health Authority, or Entity whose application to implement a Low Income Health Program (LIHP) had been approved. Completed deliverables contain the information that the County, Health Authority, or Entity provided to DHCS to demonstrate compliance with federal requirements. After completion of the authorization process, the finalized deliverables and LIHP contract are submitted to Centers for Medicare & Medicaid Services (CMS) for approval.
