Available below are the twenty-five deliverable requests that were distributed as part of the LIHP authorization process.  Completed deliverables contain the information that local LIHPs must provide to DHCS to demonstrate compliance with federal requirements.

·    DELIVERABLES REQUEST #1 - This email presented the first 24 deliverable requests, and instructions for their completion.  Included are later deliverables new to the process since the 10 legacy LIHPs completed their authorization process.  Additionally, some of the deliverables had been revised from their original form.
