LEA Targeted Case Management Labor Survey
LEA providers that want to be reimbursed for targeted case management services must complete the Targeted Case Management Labor Survey. This survey is used by DHCS to determine the TCM reimbursement rate (low, medium or high). This survey must be submitted to DHCS prior to submitting TCM claims.
