Provider Application and Validation for Enrollment​

Welcome to the Provider Application and Validation for Enrollment (PAVE) page. The PAVE portal is the Provider Enrollment Division's (PED) web-based application designed to simplify and accelerate enrollment processes. Providers can utilize the portal to complete and submit applications, report changes to existing enrollments, and respond to PED-initiated requests for continued enrollment or revalidation. PAVE features secure login, document uploading, electronic signature, application progress tracking, intuitive guidance, social collaboration and much more.

PAVE will be implemented in a series of releases to include additional provider types and enrollment actions. Visit this page periodically for updated PAVE information and access additional enrollment and PAVE resources. Please note that the current release version of the PAVE system does not support the submission of applications from new out of state providers. Please see the current release schedule to ensure your Provider Type and Enrollment Action is available.
