Targeted Case Management (TCM)

The TCM program reimburses participating counties for the federal share of costs (typically 50%) for case management services provided to Medi-Cal beneficiaries in six specific target populations. The target populations include: 1) Public Health; 2) Outpatient Clinics; 3) Public Guardian; 4) Aging and Adult Services; 5) Adult Probation; and 6) Community. Participating counties use their CPEs to draw down federal funds.
