Becoming a Waiver Provider

Participation for Home Health Agencies and Hospices in the Pediatric Palliative Care Waiver:

If your facility is at all interested in participating in this program and provides services in one or more of the 25 eligible counties (see below) please reply by email as soon as possible. Indicating interest does not obligate participation. This will help us to identify geographic areas that would be able to support a waiver site. Alameda, Butte, Contra Costa, Fresno, Humboldt, Kern, Los Angeles, Marin, Mendocino, Monterey, Orange, Placer, Riverside, Sacramento, San Bernardino, San Diego, San Francisco, San Joaquin, San Luis Obispo, Santa Clara, Santa Cruz, Sonoma, Stanislaus, Tulare and Ventura
