Coordinated Care Management Project - Seniors & Persons with Disabilities (RFP 07-00610)

The Department of Health Care Services (DHCS) is releasing Request for Proposal (RFP) 07-00610 to secure a Contractor to provide Coordinated Care Management Program (CCMP) services to fee-for-service (FFS) Medi-Cal Seniors and Persons with Disabilities (SPD) who have chronic conditions or who may be seriously ill and near end of life who are not receiving Medicare benefits. Chronic conditions that will be addressed include, but are not limited to: Cancer, Cerebrovascular Disease, Asthma, Chronic Obstructive Pulmonary Disease, Congestive Heart Failure, Coronary Artery Disease, Diabetes, Hypertension, Arthritis, Obesity and Substance Abuse.  Expected outcomes include, but are not limited to, improved healthcare, enhanced end-of-life care, and measurable cost effectiveness. 

The Office of Medi-Cal Procurement intends to make a single contract award to the most responsive and responsible firm earning the highest score.  This procurement is open to all eligible firms and/or individuals that meet the qualification requirements, including a county, a city and county, a consortium of more than one county, a health authority, commercial businesses, nonprofit organizations, State or public universities (including auxiliary organizations) and other entities.
