Alameda County
The Alameda County Excellence (ACE) program will provide an enhanced health benefits plan to all eligible and enrolled residents of Alameda County with incomes at or below 200 percent of the federal poverty level (FPL). This plan emphasizes chronic disease management through preventive primary care at a designated medical home. The target population is the high-cost, chronically ill. The goal of ACE is to coordinate access and delivery of services, and to focus the design of care to reflect the needs of chronically ill enrollees and those with sufficient risk-factors that may lead to an additional chronic condition.
