Coordinated Care Management

The Coordinated Care Management (CCM) Programs consist of two new pilot programs authorized in Fiscal Year 2005-06.  CCM Program services are currently in the procurement phase. Both CCM pilot programs are designed to improve healthcare outcomes and achieve cost containment in the fee-for-service (FFS) Medi-Cal System.  The following is a brief description of the services:

CCM I -  Focuses on Seniors and Persons with Disabilities (SPD) who have chronic conditions or who may be seriously ill and near the end of life.   

CCM II - Focuses on Persons with Chronic Conditions and Serious Mental Illnesses.
