Systems of Care Division

The Systems of Care Division (SCD) creates effective and efficient comprehensive systems for vulnerable populations with chronic conditions to better improve their health care options and reduce health care costs.  SCD is comprised of two major branches:  Medical Case Management (MCM) and Children's Medical Services (CMS).  MCM is designed to provide coordinated care for Medi-Cal fee-for-service who are chronically or catastrophically ill. CMS is responsible for administering seven programs.
