How to Submit a Claim

CHDP health assessment services are billed on the Confidential Screening/Billing Report (PM 160) claim form (version 8).  For more information refer to the CHDP Provider Manual. 

PM160’s (green and brown) can be ordered from the Children's Medical Services Catalog.  The forms are no longer imprinted with provider information.
