To be added to the Partnership's database or to make changes to your address, phone, company affiliation, etc. for those already on the database, click here to access the Database Entry Form. For your convenience, this form can be printed, completed and faxed to us at (916) 552-8989 or you can complete the form online and then print and fax.  At this time, we are unable to receive copies of this completed form via e-mail.  

NOTE: Please keep in mind that, to place an order for materials you must; 1)  be a Partnership certified agent and 2) be on our database or fully complete the Database Entry Form
