Developmentally Disabled/Continuous Nursing Care (DD/CNC) Program

The DD-CNC program provides continuous nursing care to medically fragile beneficiaries in the least restrictive setting (avoiding more restrictive care in hospitals, sub acute and developmental centers). Forty-five (45) clients reside in homes located in Santa Rosa, San Bruno, Fresno, Northridge, Gardena, Desert Hot Springs and Sylmar. 

Waiver eligibility criteria require that participants be Medi-Cal eligible, certified by a Regional Center as developmentally disabled, enrolled in the Regional Center, exhibiting medical necessity for 24-hour continuous skilled nursing care and free of any clinically active communicable disease.  Participants must also meet specific minimum medical criteria as described in the waiver.
