HCBS Advisory Workgroup Series

Advisory Group #1:  Enhancing California Community Transitions (CCT) Delivery 

This is the first in a series of five advisory workgroups being convened by the Department of Health Care Services (DHCS) to obtain input from consumers, their family members, and interested stakeholders with experience and expertise relative to specific initiatives impacting the expansion of providing HCBS to Medi-Cal beneficiaries transitioning out of long-term institutions to live and receive care in the community.  Advisory Group #1 will to provide recommended solutions to the Department DHCS on ways to strengthen ongoing CCT operations, identify opportunities to align CCT with the Final Rule, enhance the beneficiary experience, improve the quality of provided services, and increase the number of CCT transitions.  Links to Workgroup #1 documents are available at the bottom of this webpage.

Meeting 4:  Enhancing CCT Redesign

Meeting Goals:

    1. Review Workgroup Charter including objectives and outcomes;

    2. Review performance measures and quality assurance improvement plan;

    3. Examine strategies to strengthen the integration of the social and medical models of care; and

    4. Discuss and provide recommendations on where additional work is needed to enhance the delivery of CCT services.

 

Meeting 3:  Ensuring Person-Centeredness

Meeting Goals:

    1. Present the guiding principles of person centeredness.

    2. Discuss how to strike a balance between individuals’ wants and needs.

    3. Examine strategies that can be used to adhere to person-centeredness when there are limited community resources/options.

    4. Discuss and provide recommendations on how to ensure person centeredness within the CCT transition process.

    4. Present the Social Model of Care, Medical Model of Care, and identify perceived exclusivity of and conflict between the two models.
