Intermediate Care Facility for the Developmentally Disabled-Continuous Nursing (ICF/DD-CN) Pilot Project

The ICF/DD-CN program provides continuous nursing care to medically fragile beneficiaries in the least restrictive setting (avoiding more restrictive care in hospitals, sub acute and developmental centers). Thirty-six (36) clients reside in six bed pilot homes located in Santa Rosa, San Bruno, Fresno, Northridge, Gardena, and Desert Hot Springs. 

Waiver eligibility criteria require that participants be Medi-Cal eligible, certified by a Regional Center as developmentally disabled enrolled in the Regional Center, exhibiting medical necessity for 24 hour continuous skilled nursing care and/or observation, and free of any clinically active communicable disease.  Participants must also meet specific minimum medical criteria as described in the waiver.  Individuals enrolled in a Medi-Cal Managed Care Plan are not eligible to participate in the ICF/DD-CN Pilot Project.
