• Question 1:  Applicants and beneficiaries for whom a matching birth record is found through the Automated Data Match will have met the citizenship verification requirement and are not required to provide additional evidence of citizenship.  Are counties required to complete the Proof of Citizenship or identity Received form for these individuals?      
• Answer 1:  Yes, but only when a document of identity is required in the circumstances described above.  Even though citizenship documentation requirements are satisfied through the automated birth record match, identity documentation requirements are not.  Therefore, the Proof of Citizenship of identity Received form must still be completed and provided to applicants and beneficiaries as proof that they submitted acceptable evidence of identity whenever an identity document is required.
