Question 1:

The Department developed a “Proof of Citizenship or Identity Received” form and is requiring counties to check a box if the documentation has a photo or other identifying information.
Can the Department modify this form to provide space for the name and birth date of the person for whom documents were provided – along with the case name and number?
Can the Department modify the form to allow Federally Qualified Health Centers and Disproportionate Share Hospitals to complete the form?
Can the Department modify the form’s photo identification check boxes to allow the county to record that the document was brought in by the parent?
Answer 1:
(posted 11/15/07): The Department is revising Enclosure 5 (Proof of Citizenship or Identity Received) to make it a receipt rather than proof of citizenship or identity.
When an applicant or beneficiary brings a document to the county, the clerk or an eligibility worker can provide the person with a receipt, which will inform the person that the county eligibility worker will make a determination of whether the documents are acceptable.
When an applicant or beneficiary brings a document to an FQHC or DSH, the FQHC or DSH can mail the receipt form and a copy of the document(s) to the county.
The Department is creating a new form to serve as proof that the document(s) are acceptable.
The county eligibility worker will make a determination of whether the documents are acceptable and directly issue or mail the new form to the person as proof that the documents have been received and are acceptable.
The person can then have a copy of the new form that shows proof that the documents were submitted and accepted.
The person can then take the new proof form to another county, if necessary, to show that the proof was submitted, if the originating county inadvertently did not code MEDS to show that the document was received.
The revised Enclosure 5 will be used by the counties as well as FQHC and DSH facilities as entities that can complete the form.
The revised Enclosure 5 will also include the name and the date of birth of the person for whom the documents were provided.
The Department will adjust the photo identification boxes and send a draft to the counties before posting the updated Enclosure 5 to the website.

(posted 04/24/08): When should the updated DRA forms be used?
Answer 2:
(posted 04/24/08): Since the release of the ACWDL 07-12, many of the DRA forms have gone through significant changes and new forms have been added. Please discontinue using the old forms and begin using the updated and new forms immediately. A new ACWDL will include all the updated and new forms. It is important to check the DHCS DRA web site or the DHCS Forms Index for the most updated forms.
