Detailed HRT Award Nomination Process
Step 1. Award Criteria
Each candidate must be a California resident, or organization that is currently engaged in, or has completed one of the following at the local level:
- A successful community-based program to promote women’s health in California;
- A community advocacy effort to improve women’s health in California; or
- An innovative research project to address California women’s health concerns.
Step 2. Required Forms and Documents
The nomination package must include the following: Nomination Form; Nomination Criteria Form; and resume or in-depth program description for agencies or programs nominated.
Please fill out the required forms and obtain the required documents as listed below. These forms and documents make up the "Nomination Package."

Step 3. Submit the Nomination Package
The deadline to submit the package is February 29, 2008.
You may submit the Nomination package three ways:
