Find of If I Qualify

Your child may qualify if he/she:

• Lives in a Waiver County
• Is eligible for full Medi-Cal benefits
• Is 20 years of age or younger
• Has a CCS eligible medical condition in one of the following catagories:
• Cancer
• Heart defects or conditions
• Cystic Fibrosis
• Respiratory failure
• Brain or head injuries/conditions
• Spinal muscular atrophy
• Muscular dystrophy: dependent on a ventilator
• Intestinal problems: dependent on IV nutrition
• Problems following a transplant
• Progressive metabolic disorders
• Severe immune disorders
• Kidney failure
• Other conditions: check with your county's CCS program
