DHCS Webinar: HPP Overview & Stakeholder Update
Held September 26, 2018, this webinar provided stakeholders with an update on HHP. The current status of the Program was provided in an overview presentation. Program staff addressed stakeholder comments and questions in the second half of the presentation.
On July 1, 2018, HHP implementation began in San Francisco county for members with eligible chronic physical conditions and substance use disorders. HHP is expected to be fully implemented across 29 counties by January 2020, and will provide services in the following core areas: comprehensive care management; care coordination (physical health, behavioral health, community-based long-term services and supports); health promotion; comprehensive transitional care; individual and family support; and referral to community-based and social support services.
