The Child Health and Disability Prevention (CHDP) program was a preventive program that delivered periodic health assessments and services to low-income children and youth in California. CHDP program provided care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services. Health assessments were provided by enrolled private physicians, local health departments, community clinics, managed care plans, and some local school districts.

Senate Bill (SB) 184 authorized the Department of Health Care Services (DHCS) to transition the CHDP program by July 1, 2024. DHCS conducted a stakeholder engagement process to inform the development of a transition plan, detailing milestones guiding the transition of the CHDP program to other existing Medi-Cal delivery systems. The CHDP Transition Plan includes the following key components: 

DHCS acknowledges that there will continue to be a portion of the Fee-for-Service (FFS) population who may not receive care coordination services following the CHDP program transition. However, individuals in FFS will continue to receive care coordination if they qualify for specialty programs. Referrals and care management or case management services for the FFS population will continue to be processed or provided by:

  • Any FFS provider;
  • Federally Qualified Health Centers (FQHCs);
  • California Children’s Services (CCS) program;
  • HCPCFC;
  • Home and Community-Based Services (HCBS) waiver providers; 
  • Maternal, Child, and Adolescent Health (MCAH) programs;
  • County social workers through Targeted Case Management (TCM); or
  • Community health worker (CHW) benefit
