The Children’s Medical Services Branch’s Provider Services Unit makes every effort to provide current and accurate directory listings.

If your Special Care Center’s directory needs to be updated, please follow the instructions on the Special Care Center Directory Update Fax Coversheet (DHS 4507).

The DHS 4507 is for use by Special Care Centers only and must be signed by the SCC medical director. As outlined in the DHS 4507, both the completed DHS 4507 and the edited SCC directory print out must be faxed to the Provider Services Unit.
