California Community Transitions Policy and Guidance Letters

Beginning February 2014, the CCT State unit will communicate revisions and/or updates to the CCT Demonstration Project through official Policy Letters (PLs).  All PLs will be sent to Local CCT Providers and posted to the Department of Health Care Services (DHCS) website at this location.

California Community Transitions- Two-Week Public Comment from September 3, 2026- September 17, 2026

DHCS is providing an opportunity for a two-week public comment period for the Money Follows the Person (MFP)/California Community Transitions (CCT) Individualized 24/7 Backup Plan Policy Letter (PL). DHCS has updated the program’s 24/7 Backup Plan PL, which will supersede the PL 15-001 24-7 Backup. The purpose of this PL is to provide guidance on the DHCS requirements for CCT Lead Organizations (LOs) establishing and maintaining individualized 24 Hour/7 Days A Week (24/7) Backup Plans for program participants.
