Please note: DHCS encourages active program providers interested in applying for licensure to set up their organization and verify their profile on the DHCS Behavioral Health Licensing and Certification Portal.
However, applications may still be submitted by email to MHLC@dhcs.ca.gov (Psychiatric Health Facility and Mental Health Rehabilitation Center applications) and PRTF@dhcs.ca.gov (Psychiatric Residential Treatment Facility applications).
Mental health certification application types will be available in the portal at a later date.
Mental Health Rehabilitation Centers
A Mental Health Rehabilitation Center is a 24-hour program which provides intensive support and rehabilitative services designed to assist persons, 18 years or older, with mental disorders who would have been placed in a state hospital or another mental health facility to develop skills to become self-sufficient and capable of increasing levels of independence and functioning.

Psychiatric Health Facilities
Psychiatric Health Facilities are licensed by the State Department of Health Care Services and provide 24-hour inpatient care for mentally disordered, incompetent, or other persons as described in Division 5 (commencing with Section 5000) or Division 6 (commencing with Section 6000) of the Welfare and Institutions Code. Care includes, but is not limited to, the following basic services: psychiatry, clinical psychology, psychiatric nursing, social work, rehabilitation, drug administration, and appropriate food services for those persons whose physical health needs can be met in an affiliated hospital or in outpatient settings.

Psychiatric Residential Treatment Facilities
Psychiatric Residential Treatment Facilities are non-hospital facilities that provide 24-hour inpatient services to Medicaid-eligible individuals under the age of 21. These inpatient services may include assessing the beneficiary’s immediate and long-range therapeutic needs, developmental priorities, and personal strengths and liabilities, assessing the potential resources of the beneficiary’s family, setting treatment objectives, and prescribing therapeutic modalities to achieve the plan’s objectives.

File a Complaint
Any person may submit a complaint to DHCS concerning the operation of an MHRC, PHF, or PRTF. To file a complaint against an MHRC, PHF, or PRTF, please fill out the online Complaint Form and click “Submit” at the bottom of the page.
A complaint may also be filed orally or in writing. The complaint should specify enough details of the alleged violation to enable DHCS to determine the date and time, who was involved, and what the violation was. If you prefer to file your complaint by phone, email, fax, or mail, please use the MHLC Branch Complaint Contact Information below. Please note, complainants will not be automatically notified of the outcome of a complaint. However, a complainant may request the outcome upon closure of the investigation, and a Public Records Act (PRA) request will be initiated on the complainant’s behalf. If you are not the complainant and would like information regarding the outcome of a complaint after it is closed, please submit a PRA request.
