Medi-Medi Plans

A Medi-Medi Plan is a type of Medicare Advantage plan. It is for people who have both Medicare and Medi-Cal, and combines Medicare and Medi-Cal benefits into one plan. For a list of Medi-Medi Plans that are available in California, visit the Medicare Medi-Cal Plan List webpage.

Medi-Medi Plans coordinate all benefits and services across both programs, including all Medicare and Medi-Cal covered services. Download the factsheet about Medi-Medi Plans.

Starting in 2026, Medi-Medi Plans will be open for enrollment in most counties in California. See the county map of 2026 new enrollment.

Medicar + Medi-Cal Plan

Medi-Medi Plans include:

  • All Medicare Part A, B, and D benefits. Also, Medi-Cal benefits.
  • One care team to coordinate care.
  • One health plan to coordinate delivery of services, including medical supplies, transportation, and long-term services and supports.
  • One set of benefits and a network of providers, including doctors, hospitals, clinics, labs, pharmacies, and medical equipment suppliers.
  • You may have extra benefits like dental, hearing, or vision coverage, in addition to what Medi-Cal covers.

Important Updates

  • 2026 Expansion Counties: Medi-Medi Plans will be newly available in 29 additional counties in 2026. That means dual eligible members in 41 counties can choose a Medi-Medi Plan in 2026. Additional information and a visual representation of the Medi-Medi Plan 2026 Expansion is available in the Medi-Medi Plan Expansion Fact Sheet.
  • Default Enrollment Pilot: DHCS has launched a Default Enrollment Pilot for a select group of Medi-Medi Plans in San Diego and San Mateo for full benefit dual eligible individuals. Learn more about the Default Enrollment Pilot.
  • D-SNP Deeming Period: Medicare Advantage D-SNPs in California are required to maintain enrollment for their members for at least a three-month deeming period upon notification that a member has lost Medi-Cal eligibility. Joining a Medicare Medi-Cal Plan

There are three ways to join a Medi-Medi Plan:

  • If already enrolled in a Medi-Cal plan, call the plan directly and ask about their Medi-Medi Plan option
  • Call Medicare at (800) MEDICARE
  • Call Health Care Options at 1-800-430-4263 (TTY 1-800-430-7077).

Beneficiaries can join a Medi-Medi Plan if they:

  • Have both Medicare Part A and B, and Medi-Cal 
  • Are 21 years or older

Where are Medi-Medi Plans Available:

Medi-Medi Plans are open for enrollment in 12 counties in 2025: Fresno, Kings, Los Angeles, Madera, Orange, Riverside, Sacramento, San Bernardino, San Diego, San Mateo, Santa Clara, and Tulare. 

Starting in 2026, Medi-Medi Plans will be open for enrollment in the following counties in California: Alameda, Alpine, Amador, Calaveras, Contra Costa, El Dorado, Imperial, Inyo, Kern, Mariposa, Merced, Mono, Monterey, San Benito, San Francisco, San Joaquin, San Luis Obispo, Santa Barbara, Santa Cruz, Stanislaus, Tuolumne, Ventura, and also at least one plan in Marin, Napa, Placer, Solano, Sonoma, Yolo, Yuba.

PACE

Program of All-Inclusive Care for the Elderly (PACE): PACE covers Medicare and Medi-Cal benefits, including prescription drugs. It will coordinate healthcare, homecare, transportation, and dental care. To find out if PACE is available in your area, and to enroll, please go to the CalPACE website

SCAN Fully-Integrated Dual Eligible Special Needs Plans (FIDE SNPs)

SCAN Connections and SCAN Connections at Home are Medicare Advantage Special Needs Plans that covers Medicare and Medi-Cal benefits, including prescription drugs. The plans will coordinate all healthcare, homecare, transportation, and related benefits. To find out if SCAN Connections at Home is available in your area, and to enroll, please go to the Scan Health Plan website

To learn more about Medi-Medi Plans, stakeholders can participate in the bi-monthly CalAIM Managed Long-Term Services and Supports and Duals Integration Workgroup, which is open to the public. The workgroup is a collaboration hub where stakeholders can offer feedback to the Department of Health Care Services (DHCS) and engage in discussions related to the policy, operations, and strategy for the D-SNP transition.
