The purpose of this All Plan Letter (APL) is to provide Medi-Cal managed care plans (MCPs) with instructions on the payment and data sharing process required for the Skilled Nursing Facility (SNF) Workforce and Quality Incentive Program (WQIP) for Rating Periods between January 1, 2023, and December 31, 2025. The Calendar Year (CY) 2023 Rating Period is referred to as Program Year (PY) 1, CY 2024 Rating Period as PY 2, and so forth.
DHCS published the final version of the SNF WQIP Technical Program Guide for PY2 (2024). Public input is requested, and comments are due to DHCS on November 1, 2024.
The SNF WQIP CY 2024 Qualifying List Spreadsheet includes the SNF WQIP qualifying facilities and the attributable National Provider Identifiers (NPIs), Health Care Access Information (HCAI) Identification (ID) for any 2024 dates of service. This report provides an evaluation of SNF WQIP PY 1 (CY2024) metrics in comparison to the established baseline performance targets. It also assesses the factors driving the facilities’ performance levels on the metrics and factors impacting their overall SNF WQIP scores and subsequent payments.

The NPIs for the 2023 WQIP Interim Payment were obtained from publicly available CDPH licensing data. All facilities should verify that the correct NPIs and Health Care Access and Information (HCAI) IDs are listed for their facility for any 2023 dates of service. If your current or previous NPI is not listed on the CY2023 SNF WQIP Days Summary SNF WQIP Qualifying Facilities List, please email the DHCS SNFWQIP Inbox at SNFWQIP@dhcs.ca.gov and complete the form provided in the automatic reply. Note that if an incorrect NPI is listed along with all correct NPIs, there is no reason to reach out to DHCS via email. Please note, only submit one form per facility.
