Existing Casualty Insurance Cases
If you have an existing case with the Casualty Insurance Section (CIS), please allow 120 days to receive an update on your lien. In order to expedite the processing of your case, we need complete information regarding the injury, treatment, and settlement.
The following information is necessary to make an accurate and timely determination of the lien:
- Beneficiary’s final date of treatment for services paid by Medi-Cal. Please note that if the beneficiary is continuing to treat with a non-Medi-Cal provider, the lien may be finalized if CIS received complete payment data.
- List of beneficiary’s treatment dates and Medi-Cal providers. If you received a current lien and found that services paid by Medi-Cal are missing, please provide information on the missing services.
- Settlement information. Click on the link below to fill out the settlement information form. Please be prepared to attach a copy of the insurance company’s release or the third party settlement agreement (must be signed).
- Source(s) of payment for the medical bills if Medi-Cal did not pay for any services.
