MCP-Hub Toolkit Informational Webinar​​ 

The Department of Health Care Services and Aurrera Health Group created a toolkit for MCPs and “hubs,” which are intermediaries between MCPs and the community-based organizations, doulas, community health workers, and other providers that address health-related social needs. The toolkit helps plans and hubs establish effective partnerships, streamline contracting, and expand provider networks to deliver coordinated care.​​ 

This webinar, hosted by DHCS, introduces the MCP-Hubs Toolkit and walks through contracting guidance, oversight tools, and program-specific considerations for Enhanced Care Management, Community Supports, community health workers, and doula services.​​
