Doula 애플리케이션 정보
Doula services include personal support to women and families throughout a woman’s pregnancy, childbirth, and postpartum experience. This includes emotional and physical support, provided during pregnancy, labor, birth, and the postpartum period. Pursuant to federal regulations, doula services must be recommended by a physician or other licensed practitioner.
Doula providers may apply for enrollment in the fee-for-service Medi-Cal program as individuals, group providers, rendering providers, or ordering/referring/prescribing (ORP) providers.
In accordance with Welfare & Institutions Code (W & I Code) Section 14043.75(b), DHCS established specific application and enrollment requirements for doula providers who apply for enrollment in the Medi-Cal program to be reimbursed for the covered services they provide to Medi-Cal beneficiaries. Doula applicants may apply to enroll in the Medi-Cal program by submitting an electronic application through the Provider Application for Validation and Enrollment (PAVE) online enrollment portal, along with all supporting documentation.

교육 과정 요구 사항

- 다음 주제를 모두 포함한 총 16시간 이상의 교육을 이수한 수료증입니다:
- 수유 지원;
- 출산 교육;
- 임신과 출산의 해부학에 대한 기초;
- 비의료적 편의 조치, 산전 지원 및 분만 지원 기술, 그리고
- 커뮤니티 리소스 목록 개발
수료증에 총 이수 시간 및 다루는 주제가 자세히 명시되어 있지 않거나 둘라 신청자가 수료증을 가지고 있지 않은 경우, 둘라 신청자는 이수한 과정의 강의 계획서 사본을 제출하고 위의 필수 주제를 다루는 과정을 만족스럽게 이수했음을 증명하는 해당 증명서를 교육 과정을 제공하는 기관의 이름, 총 이수 시간 및 과정 완료 날짜와 함께 전자 양식 신청서 내에 작성해야 합니다.
- 출산 도우미의 자격으로 세 번의 출산에 지원을 제공했음을 증명하는 e-Form 신청서 내 증빙 서류.
체험 경로 요구 사항

1. 최소 5년 동안 유급 또는 자원봉사 자격으로 둘라의 자격으로 서비스를 제공했음을 증명하는 e-Form 신청서 내 증빙 서류. 둘라로서 5년의 경력은 신청서를 제출한 날로부터 최근 7년 이내에 발생한 경력이어야 합니다.
2. 본 규정 제공자 게시판에 제공된 추천서 템플릿을 사용하여 의사, 면허를 소지한 행동 건강 제공자, 전문간호사, 조산사, 면허를 소지한 조산사, 등록된 둘라 또는 지역사회 기반 단체 중 한 곳에서 작성한 고객 추천서 또는 전문가 추천서 3통입니다. 편지는 최근 7년 이내에 작성된 것이어야 합니다. 허가된 제공자, 커뮤니티 기반 조직 또는 등록된 둘라가 보낸 편지여야 합니다.
둘라 지원자를 위한 추천서 템플릿
모든 둘라에 필요한 서류
1. 제공자의 운전면허증 또는 주에서 발급한 신분증 (미국 50개 주 또는 컬럼비아 특별구 내에서 발급) 또는 신청자 또는 제공자를 법적으로 구속할 권한이 있는 신청서에 서명하는 사람.
2. 현재 국세청(IRS)에서 생성한 문서를 제출하여 연방 고용주 식별 번호 (FEIN)를 확인합니다. 허용되는 서류에는 IRS에서 작성한 Letter 147-C, IRS에서 작성한 양식 941(고용주의 분기별 연방 세금 신고서), IRS에서 작성한 양식 8109-C(입금 쿠폰) 또는 IRS에서 작성한 양식 SS-4(FEIN 할당에 대한 공식 확인 통지서만 해당)가 있습니다. 참고: 신청서에 기재된 신청자 또는 제공자의 법적 이름이 IRS 문서에 기재된 이름과 일치해야 합니다. 자세한 내용은 IRS를 방문하거나 (800) 829-4933번으로 문의하시기 바랍니다.
3. Local Business License, Tax Certificate, and Permit for any city and/or county where business activities are conducted. Note: The name and business address of the applicant or provider on the application must match the business name and business address on all local licenses and permits. 둘라 제공자를 위한 참고 사항
Doula 제공업체는 CCR, 제22장 51000.60절에 명시된 사업장 요건에서 면제됩니다.
Additionally, doulas may use an “administrative location” for the purposes of reporting a business address in the e-Form application. This may be the same as their mailing address but it cannot be a post office box. This address will appear in directories and in the California Health and Human Services Open Data Portal. For the purposes of doula enrollment, an “administrative location” is defined as the physical location associated with the doula’s operations, which can include where doulas are dispatched or based. They are not required to provide services at the administrative location.
