The July 23, 2015, webinar is to address the most frequently asked questions regarding the Health Homes for Patients with Complex Needs California Concept Paper Version 2.0.  We will also respond to stakeholder comments and answer questions submitted during the webinar.

The Health Homes Program is an optional Medicaid benefit that will coordinate access to medical and behavioral health services and long-term services and support, and provide linkages to community social supports.
