Rendering Provider Application Information
Rendering providers are required to submit their applications via PAVE (Provider Application and Validation for Enrollment).
The following provider types can apply as rendering providers:
Acupuncturist
Audiologist
Chiropractor
Clinical Social Worker
Dispensing Optician
Doctor of Medicine
Doctor of Osteopathy
Hearing Aid Dispenser
Nurse Anesthetist
Nurse Midwife (if joining nurse midwife group)
Nurse Practitioner (if joining nurse practitioner group)
Occupational Therapist
Optometrist
Orthotist
Physical Therapist
Podiatrist
Portable X-Ray Provider
Prosthetist
Psychologist
Respiratory Care Practitioner
Speech Therapist
Licensing
Prior to applying to Medi-Cal, first check the Licensing requirements on the licensing board website that applies to your specific provider type.
Required Documents for Rendering Providers
Next, gather the required documents listed below, as applicable, in order to upload them into PAVE as you complete your PAVE application. Please ensure the uploaded documents are legible.
1. Driver’s License or state-issued identification card (issued within the 50 United States or the District of Columbia) of the rendering applicant and the Driver’s License or State-issued identification of the supervising physician and employing physician.
2. A copy of the rendering applicant/provider’s Professional License.
3. Certificate of Professional Liability Insurance (as group member, individual, or both) in an amount of not less than $100,000 per claim and minimum annual aggregate of $300,000.
4. Anesthesia Permit, Conscious Sedation Permit and/or DEA Certificate, if applicable.
