Notice of Privacy Practices (NPP)

There are two formats for the NPPs.

  • The regular format is a “Brochure Format.”  It must be printed out, photocopied back-to-back, and folded in thirds or fourths (depending on the number of columns). 
  • The “Large Print” format prints out on 8½ x 11 inch paper and requires no special folding.
