​​​Hearing Conservation Program

The California Department of Health Care Services (DHCS), Children’s Medical Services (CMS) has implemented a statewide comprehensive Hearing Conservation Program (HCP).  The HCP helps to identify hearing loss in preschoolers to 21 years of age in Public Schools, Child Health and Disability Prevention (CHDP) programs and other state supported programs.

Please Note: PM 100 Forms

  • All school districts must collect all hearing screening results from each school (including charter schools) in their district and submit one PM 100 with the total combined results.  Individual PM 100's from schools will no longer be accepted.
  • All school districts are required to submit a PM 100, Annual Report of Hearing Testing Form regardless whether a waiver for the 10th and/or 11th grade was requested. 
  • The PM 100 Annual Report of Hearing Testing is due by June 30th of each year.  
  • Please use forms provided by the HCP only.
