SCAN Health Plan is a Medicare organization that offers Medicare Advantage (MA) plans, including California’s only Fully Integrated Dual Eligible Special Needs Plan (FIDE-SNP). The SCAN FIDE-SNP serves individuals dually eligible for Medicare and Medi-Cal in Los Angeles, Riverside, San Bernardino, and San Diego counties. More specifically, to enroll in the SCAN FIDE-SNP, individuals must have both Medicare Part A and Part B, have full scope Medi Cal with no share of cost, be at least 65 years old and live in one of the counties listed above. 

The two SCAN FIDE-SNP Plans are described below.

• SCAN Connections is open to all dually eligible members that meet the criteria listed above.
• SCAN Connections at Home is open to all dually eligible members that meet the criteria listed above and meet nursing facility level of care. 

Both of the SCAN FIDE-SNP Plans integrate care by providing a full range of Medicare and Medi-Cal benefits under a single managed care organization. These plans cover all healthcare, homecare, transportation, and related benefits, including home and community-based services. 
Members enrolled in SCAN Connections and SCAN Connections at Home receive in-home care and care coordination through SCAN, and may not also receive In-Home Supportive Services (IHSS), or be enrolled in the Assisted Living Waiver (ALW), the Home and Community Based Alternatives (HCBA) Waiver, or Multipurpose Senior Services Program (MSSP). However, members in other SCAN Medicare Advantage plans may be eligible for IHSS or the ALW, HCBA, and MSSP waivers.
