​​​​​​ADHC Transition: Beneficiary Notices​

Notice 1   ​​

This notice is for ADHC members who are not able to join a managed care health plan.  The notice explains the change to ADHC services, how to get more information about other options, and provides a telephone number and website.  The notice will be sent in English and all appropriate threshold languages.

Notice 2.1​

This notice will be mailed to ADHC members who are both Medicare and Medi-Cal (dual) beneficiaries and who are able to join a managed care health plan.  The notice explains the changes to ADHC services and informs beneficiaries that they will be enrolled in an available managed care health plan on October 1, 2011, unless they either choose one of the available managed care health plans or choose not to be enrolled in a managed care health ​plan.  The notice has been sent in English and the enrollment packet in appropriate threshold languages. A sample voluntary enrollment packet for Los Angeles County is attached to the notice.

Notice 2.1 - Regional Center Consumers

This notice will be mailed to ADHC members who are both Medicare and Medi-Cal (dual) beneficiaries and who are able to join a managed care health plan.  The notice explains the changes to ADHC services and informs beneficiaries that they will be enrolled in an available managed care health plan on November 1, 2011, unless they either choose one of the available managed care health plans or choose not to be enrolled in a managed care health plan.  The notice has been sent in English and the enrollment packet in appropriate threshold languages. A sample voluntary enrollment packet for Los Angeles County is attached to the notice.

Notice 2.2 

​This notice will be mailed to ADHC members who are both Medicare and Medi-Cal (dual) beneficiaries and are able to join a managed care health plan to remind such beneficiaries of the deadline to select a managed care health plan or chooses not to be enrolled in a managed care health plan. This notice will be sent in English and appropriate threshold languages.

Notice 3

This notice is for ADHC members who are eligible for Medi-Cal only, classified as Seniors and Persons with Disabilities (SPDs), are able to join a managed care health plan, have previously been notified that they will have to join a managed care health plan during their birth month, and are within 90-days of their birth month. The notice explains the change to ADHC services, how to enroll in a managed care health plan, and includes important telephone numbers for more information.  This notice will be sent in English and appropriate threshold languages.

Notice 4

This notice is for ADHC members who are eligible for Medi-Cal only, classified as Seniors and Persons with Disabilities (SPDs), are able to join a managed care health plan, and have not been previously been notified that they will have to join a managed care health plan during their birth month. The notice explains the change to ADHC services, how to enroll in a managed care health plan,, and includes important telephone numbers for more information.  The notice will include an enrollment packet and will be sent in English and all appropriate threshold languages. A sample voluntary enrollment packet for Los Angeles County is attached to the notice.

Notice 5.1

This notice is for ADHC is to ADHC members enrolled in a managed care health plan that is not a COHS, explaining ADHC changes and advising members to contact their health plan for assistance. The notice will be sent in English and all threshold languages.

Notice 5.2

​This notice is to ADHC members enrolled in a COHS managed care health plan explaining ADHC changes and advising members to contact their health plan for assistance. The notice lists the phone numbers of all the COHS plans. The notice will be sent in English and all threshold languages.

Voluntary Enrollment Packet ​

This is a sample voluntary enrollment packet for Los Angeles County that was sent to ADHC beneficiaries in counties where managed care is available. The packet was sent in appropriate threshold languages and specific to each County.
