Quality and Population Health Management
The Quality and Population Health Management program (QPHM) leads clinical quality, health equity, and population health work at DHCS.
QPHM Vision:
All Californians get the quality care they need for optimal health.
QPHM Mission:
To improve equity, access, and quality of whole person care for all Medi-Cal members.
QPHM Core Values:
Collaboration
We value teams working together inside and outside our organization that create value by improving innovation, stakeholder relationships, and efficiency. We value partnerships. We foster the public’s trust through open communication and work in a cooperative, respectful, and courteous manner.
Equity
Equity is achieved when every person has the opportunity to “attain his or her full potential” and no one is “disadvantaged from achieving this potential because of social position or other circumstances.”
Excellence
We have a passion for quality and strive for continuous quality improvement of our programs, services, and processes through employee empowerment and professional development.
