Medi-Cal Connect 

Medi-Cal Connect is a statewide data analytics solution and tool for population health management (PHM). It is designed to strengthen coordination across the Medi-Cal program, close gaps in services, and improve member health outcomes.

Medi-Cal Connect supports the DHCS PHM Initiative, which is a component of CalAIM. Together, these efforts are helping to build a stronger, more connected health care system for all Californians.

Medi-Cal Connect aims to:

  • Inform policymaking.
  • Provide population-level insights.
  • Support personalized whole-person care.
  • Improve member experience.

How Does Medi-Cal Connect Work?

Medi-Cal Connect is a first-of-its-kind data analytics tool that integrates information from trusted partners, like Medi-Cal health plans and other state agencies.

By combining data from different sources, Medi-Cal Connect provides an expanded view of member health needs and population-level insights to help inform DHCS policy and programs. ​Its Risk Segmentation, Stratification, and Tier (RSST) Algorithm is a statewide risk tiering tool that incorporates physical and behavioral health and social risk factors. RSST provides a data-driven method for identifying Medi-Cal members who may be at increased risk of poor outcomes or underutilization of essential services, and who may benefit from additional outreach or care coordination.​

The Medi-Cal Connect service is just beginning and will continue to evolve and improve based on real-world user input.
