Medi-Cal Connect is a statewide data analytics solution and tool for population health management (PHM). It is designed to strengthen coordination across the Medi-Cal program, close gaps in services, and improve member health outcomes.
Medi-Cal Connect supports the DHCS PHM Initiative, which is a component of CalAIM. Together, these efforts are helping to build a stronger, more connected health care system for all Californians.
By combining data from different sources, Medi-Cal Connect provides an expanded view of member health needs and population-level insights.
