The links below include previous versions of the Dual-Eligible Special Needs Plan (D-SNP) dashboard. These are a continuation of the Cal MediConnect dashboard, which includes enrollment, demographic, and federal and state-specific measures of D-SNPs. Additionally, the links below include D-SNP quality measure data not included in the D-SNP Business Intelligence (BI) dashboard and other reports with duals-specific data. D-SNP Reported Data
DHCS has state-specific reporting requirements for D-SNPs. More information on these requirements is available in the D-SNP Policy Guide and on the D-SNP Quality and Data Reporting webpage.
In addition to data published in the D-SNP BI Dashboard, the following data tables display plan reported data for select Contract Year(s) state-specific measures. As available, additional measures will be posted on this page. The data will be included in future updates to the D-SNP BI Dashboard.
