There are currently 41 local agencies statewide under contract with the California Department of Aging, provide social and health care management for frail elderly clients who are certifiable for placement in a nursing facility but who wish to remain in the community. The goal of the MSSP is to arrange for and monitor the use of community services to prevent or delay premature institutional placement of these frail clients. Clients eligible for the program must be 65 years of age or older, live within an MSSP site service area, be able to be served within MSSP’s cost limitations, be appropriate for care management services, be currently eligible for Medi-Cal, and be certified or certifiable for placement in a nursing facility.  Services provided by MSSP include: adult-day care / support center, housing assistance, chore and personal care assistance, protective supervision, care management, respite, transportation, meal services, social services, communication services.​​

MSSP-ի հրաժարման հաստատման հայտարարություն​​ 

Առողջապահական խնամքի ծառայությունների վարչությունը (DHCS) և Կալիֆորնիայի ծերացման վարչությունը ուրախ են հայտարարել 1915(c) Բազմաֆունկցիոնալ Ծառայությունների Ծառայությունների ծրագրի, Տնային և Համայնքի վրա հիմնված ծառայությունների հրաժարում, Վերահսկիչ համար 0141.05.00:  Ուրբաթ օրը՝ դեկտեմբերի 19, 2014, DHCS-ը պաշտոնական ծանուցում է ստացել Medicare և Medicaid ծառայությունների կենտրոններից, որ MSSP-ի հրաժարումը հաստատվել է հուլիսի 1, 2014 մինչև հունիս 30, 2019 ժամանակահատվածի համար:  MSSP-ը կշարունակի տրամադրել սոցիալական և առողջապահական խնամքի կառավարման ծառայություններ և ձեռք բերված հրաժարման ծառայություններ թուլացած տարեցների համար՝ որպես ինստիտուցիոնալացման ծախսերի արդյունավետ այլընտրանք:​​
