이중 수혜자 통합 의료 시범 사업 – 캘리포니아 메디커넥트
Cal MediConnect는 메디케어와 메디칼 모두에 자격이 있는 사람들에게 통합적인 의료 서비스를 제공하기 위해 설계된 의료 프로그램입니다. 두 프로그램의 장점을 하나의 건강 보험으로 통합하고, 추가적인 진료 조정 혜택을 제공했습니다. 1 년 1월 2023 에 Cal MediConnect 플랜을 제공했던 회사는 이 인구를 위한 의료 서비스 제공을 계속 조정하는 Medicare Medi-Cal 플랜(Medi-Medi 플랜 또는 MMP)을 제공하기 시작했습니다.
이 데모는 캘리포니아의 Medi-Cal 프로그램과 메디케이드 서비스 센터( Medicare ) 및 메디케이드 서비스(CMS) 간의 파트너십으로 진행되었으며, 이중 자격 수혜자를 위한 원활한 서비스 제공 환경을 조성하여 치료 조정 개선, 건강 결과 개선 및 보다 효율적인 전달 시스템을 궁극적인 목표로 삼았습니다.
Cal MediConnect was a part of California’s larger Coordinated Care Initiative (CCI). Building on many years of stakeholder discussions, the CCI was enacted in July 2012 through SB 1008 (Chapter 33, Statutes of 2012) and SB 1036 (Chapter 45, Statutes of 2012). Further updates and clarifications to this initiative were enacted in June 2013 through SB 94 (Chapter 37, Statutes of 2013).
In 2014, the Cal MediConnect program was ultimately implemented in seven counties: Los Angeles, Orange, Riverside, San Bernardino, San Diego, San Mateo and Santa Clara. The transition to Medicare Medi-Cal plans in 2023 continues the goal of coordinated care for this population.
