Passage of the Coordinated Care Initiative (CCI) in July 2012 marked an important step toward transforming California’s Medi-Cal (Medicaid) care delivery system to better serve the state’s low-income seniors and persons with disabilities. Building upon many years of stakeholder discussions, the CCI begins the process of integrating delivery of medical, behavioral, and long-term care services and provides a road map to integrate Medicare and Medi-Cal for beneficiaries of both programs, called “dual eligible” beneficiaries.  

이해관계자와 의료 소비자가 참여한 공개적인 절차를 통해 만들어진 CCI는 상원 법안 1008호(2012년 법령 제33장) 와 상원 법안 1036호(2012년 법령 제45장) 를 통해 제정되었습니다.

The CCI was implemented in seven counties beginning in 2014. The seven counties are Los Angeles, Orange, Riverside, San Bernardino, San Diego, San Mateo, and Santa Clara.

코디네이트 케어 이니셔티브의 주요 부분

• Cal MediConnect: 이중 자격 수혜자가 단일 조직화된 전달 시스템을 통해 조정된 의료, 행동 건강, 장기 시설, 가정 및 지역 사회 기반 서비스를 받을 수 있는 자발적 의료 보험입니다. Cal MediConnect Medicare 및 Medi-Cal 혜택과 추가 케어 코디네이션 혜택이 결합되어 있습니다.  
• 관리되는 장기 지원 및 Medi-Cal 서비스(MLTSS): CCI 카운티의 이중 자격 Medi-Cal 수혜자를 포함한 모든 수혜자는 의료 보험에 Medi-Cal Managed Care Medi-Cal LTSS Medicare 가입해야 및 랩 어라운드 혜택을 포함한 혜택을 받을 수 있습니다.

What’s New with Cal MediConnect

캘리포니아 메디칼 발전 및 혁신(CalAIM) 계획에 따라 1월 1, 2023 부터 Cal MediConnect 회원은 MMP 또는 Medi-Medi 플랜으로 전환됩니다. MMP는 캘리포니아 주에서만 사용되는 프로그램 명칭으로, 특수 요구 아동을 위한 전용 등록 이중 자격 플랜(EAE D-SNP)을 의미합니다. 전적으로 연계된 등록 제도를 통해 수혜자는 메디케어 혜택을 위한 D-SNP와 메디칼 혜택을 위한 메디칼 관리형 의료 플랜에 등록할 수 있으며, 두 플랜 모두 더 나은 의료 서비스 조정 및 통합을 위해 동일한 모기관에서 운영됩니다. DHCS는 건강 보험 회사, 이해 관계자 및 메디케어 및 메디케이드 서비스 센터(CMS)와 협력하여 통합 의료 서비스를 주 전역으로 확대하고 전환할 것입니다.  이 정책은 CalAIM 계획과 캘리포니아 노인 종합 계획 모두에서 주 전체의 목표인 향상된 의료 통합 및 환자 중심 의료를 달성하는 데 도움이 되도록 마련되었습니다.
