The 1115 “Bridge to Reform” Waiver (Waiver) renewal of November 2010 was intended to identify and test alternative health care delivery models for the California Children’s Services (CCS) program. A CCS Demonstration Project (DP) was pursued to test the efficacy of transitioning the CCS program from a fee-for-service based health care delivery model to an organized health care delivery model with a capitated payment. The CCS DP tests new approaches to improve the quality of care provided to children (up to 21 years of age) with special health care needs and focuses on identifying and solving roadblocks to access to care and ensuring families receive appropriate health care services for their child or youth.

4월 1 에 2013 San Mateo 카운티 보건 시스템과 파트너십을 맺은 San Mateo 건강 계획(HPSM)은 면제에 따른 최초의 운영 CCS DP 파일럿이 되었습니다. DHCS는 향상된 진료 조정을 통해 카운티의 CCS 프로그램을 개선하기 위해 완전 위험 부담 방식의 메디칼 관리형 의료 플랜 모델 하에서 CCS DP 시범 사업에 참여할 HPSM을 선정했습니다. HPSM은 약 1,400명의 CCS 수혜 대상자에게 포괄적인 의료 서비스를 제공하며, 정기적인 건강 검진, 예방 접종, CCS 수혜 대상 질환과 관련 없는 1차 의료 서비스 및 전문 의료 서비스를 포함하여 아동의 전반적인 건강을 위한 모든 범위의 의료 서비스를 관리 및 조정합니다.
