The latest news about the Justice-Involved Reentry Initiative and the significant steps California is taking to address poor health outcomes in Justice-Involved individuals – people who are now, or have spent time, in jails, youth correctional facilities, or prisons.

DHCS ਨੂੰ ਰੀਐਂਟਰੀ ਡੈਮੋਨਸਟ੍ਰੇਸ਼ਨ ਇਨੀਸ਼ੀਏਟਿਵ ਲਾਗੂਕਰਨ ਯੋਜਨਾ ਦੀ ਪ੍ਰਵਾਨਗੀ ਮਿਲੀ
- In October 2024, DHCS received approval from CMS of its Reentry Demonstration Implementation Plan.
- ਲਾਗੂਕਰਨ ਯੋਜਨਾ, ਅਕਤੂਬਰ 1, 2024 ਤੋਂ ਪ੍ਰਭਾਵੀ, ਹੇਠ ਲਿਖੇ ਪੰਜ ਸੈਕਸ਼ਨ 1115 ਪ੍ਰਦਰਸ਼ਨ ਛੋਟ ਮੀਲ ਪੱਥਰਾਂ ਦੇ ਆਲੇ-ਦੁਆਲੇ ਸੰਗਠਿਤ ਕੀਤੀ ਗਈ ਹੈ:
- ਕੈਦ ਕੀਤੇ ਗਏ ਵਿਅਕਤੀਆਂ ਲਈ ਕਵਰੇਜ ਵਧਾਉਣਾ ਅਤੇ ਕਵਰੇਜ ਦੀ ਨਿਰੰਤਰਤਾ ਨੂੰ ਯਕੀਨੀ ਬਣਾਉਣਾ।
- ਭਾਈਚਾਰੇ ਵਿੱਚ ਵਾਪਸੀ 'ਤੇ ਦੇਖਭਾਲ ਤਬਦੀਲੀਆਂ ਨੂੰ ਬਿਹਤਰ ਬਣਾਉਣ ਲਈ ਕੈਦ ਕੀਤੇ ਗਏ ਵਿਅਕਤੀਆਂ ਲਈ ਪ੍ਰੀ-ਰਿਹਾਈ ਸੇਵਾਵਾਂ ਦੇ ਘੱਟੋ-ਘੱਟ ਸੈੱਟ ਨੂੰ ਕਵਰ ਕਰਨਾ ਅਤੇ ਉਹਨਾਂ ਤੱਕ ਪਹੁੰਚ ਯਕੀਨੀ ਬਣਾਉਣਾ।
- ਦੇਖਭਾਲ ਦੀ ਨਿਰੰਤਰਤਾ ਨੂੰ ਉਤਸ਼ਾਹਿਤ ਕਰਨਾ।
- ਮੁੜ-ਦਾਖਲ ਹੋਣ ਵਾਲੀ ਆਬਾਦੀ ਦੀਆਂ ਜ਼ਰੂਰਤਾਂ ਨੂੰ ਪੂਰਾ ਕਰਨ ਲਈ ਰਿਹਾਈ ਤੋਂ ਬਾਅਦ ਉਪਲਬਧ ਸੇਵਾਵਾਂ ਨਾਲ ਜੁੜਨਾ।
- ਕਰਾਸ-ਸਿਸਟਮ ਸਹਿਯੋਗ ਨੂੰ ਯਕੀਨੀ ਬਣਾਉਣਾ।
DHCS ਨੇ ਰੀਐਂਟਰੀ ਇਨੀਸ਼ੀਏਟਿਵ ਲਰਨਿੰਗ ਕੋਲੈਬੋਰੇਟਿਵ ਸੀਰੀਜ਼ ਲਾਂਚ ਕੀਤੀ

- The Data Sharing Authorization Guidance “Reentry Initiative” Toolkit is designed to complement the California Advancing and Innovating Medi-Cal (CalAIM) Data Sharing Authorization Guidance (DSAG) by illustrating real world scenarios that will help Medi-Cal Partners navigate data privacy and data sharing laws and regulations related to CalAIM.

- On January 6, 2025, DHCS opened the PATH CITED Round 4 application window. The PATH CITED initiative provides funding to build the capacity and infrastructure of on-the-ground partners, including community-based organizations (CBO), hospitals, county agencies, Tribes, and others, to successfully participate in Medi-Cal. DHCS has decided to extend the Round 4 application deadline through July 2025. This announcement supersedes previous deadline communications, and updated materials are forthcoming.
