In January 2007, the California Department of Health Care Services (DHCS) was awarded special federal grant number 1LICMS300149 to implement a Money Follows the Person (MFP) Rebalancing Demonstration, known as “California Community Transitions” (CCT). 
DHCS works with designated CCT Lead Organizations (LOs) to implement the CCT program. Facility stays for short-term rehabilitation services reimbursed by Medicare are not counted toward the 60-day required period.  Individuals are eligible for this program if they have a physical and/or mental disability and have resided in a state-licensed health care facility for a period of 60 consecutive days or longer (e.g., with at least 1-day of Medi-Cal covered services). There is no age restriction for this program.
NOTE: Facility stays for short-term rehabilitation services reimbursed by Medicare are not counted toward the 60-day required period.​​

• Employ or contract with transition coordinators who work directly with willing and eligible individuals, support networks, and providers to facilitate and monitor beneficiaries’ transitions from facilities to the community settings of their choice.​​ 
• Conduct assessments to determine the participant’s level of care.​​

State Funded California Community Transitions – Like  Program – Last Day to Enroll Participants Using this Criteria is December 31, 2025​​ 

ਜੁਲਾਈ 27, 2021 ਨੂੰ, ਅਸੈਂਬਲੀ ਬਿੱਲ (AB) 133 ਨੂੰ ਕਾਨੂੰਨ ਵਿੱਚ ਸ਼ਾਮਲ ਕੀਤਾ ਗਿਆ।​​ 

AB 133 ਦੀ ਪ੍ਰਵਾਨਗੀ ਇੱਕ ਰਾਜ-ਫੰਡ ਪ੍ਰਾਪਤ, CCT-ਵਰਗੇ ਪ੍ਰੋਗਰਾਮ ਨੂੰ ਲਾਗੂ ਕਰਨ ਦੀ ਆਗਿਆ ਦਿੰਦੀ ਹੈ ਜਿਸਨੂੰ ਮਨੀ ਫਾਲੋਜ਼ ਦ ਪਰਸਨ (MFP) ਡੈਮੋਸਟ੍ਰੇਸ਼ਨ ਅਧਿਕਾਰਤ ਕਾਨੂੰਨ ਵਿੱਚ ਸੰਘੀ ਸੋਧ ਕਾਰਨ ਅਵੈਧ ਕਰਾਰ ਦਿੱਤਾ ਗਿਆ ਸੀ।  AB 133, ਇੱਕ ਇਨਪੇਸ਼ੈਂਟ ਸਹੂਲਤ ਵਿੱਚ ਰਿਹਾਇਸ਼ ਦੀ ਲੋੜੀਂਦੀ ਮਿਆਦ ਨੂੰ 90 ਤੋਂ ਘਟਾ ਕੇ 60 ਦਿਨਾਂ ਤੱਕ, ਰਾਜ ਦੇ ਕਾਨੂੰਨ ਨੂੰ ਸੋਧੇ ਹੋਏ ਸੰਘੀ ਕਾਨੂੰਨ ਨਾਲ ਜੋੜਦਾ ਹੈ। ਰਾਜ-ਫੰਡ ਪ੍ਰਾਪਤ, CCT-ਵਰਗਾ ਪ੍ਰੋਗਰਾਮ CCT ਲੀਡ ਸੰਗਠਨਾਂ ਨੂੰ Medi-Cal ਲਾਭਪਾਤਰੀਆਂ ਨੂੰ ਤਬਦੀਲੀ ਸੇਵਾਵਾਂ ਪ੍ਰਦਾਨ ਕਰਨ ਦੀ ਆਗਿਆ ਦਿੰਦਾ ਹੈ ਜੋ ਅਜੇ ਤੱਕ ਸੰਘੀ, MFP ਰਿਹਾਇਸ਼ੀ ਯੋਗਤਾ ਮਾਪਦੰਡਾਂ ਨੂੰ ਪੂਰਾ ਨਹੀਂ ਕਰਦੇ ਹਨ, ਤਾਂ ਜੋ COVID-19 ਜਨਤਕ ਸਿਹਤ ਐਮਰਜੈਂਸੀ (PHE) ਦੌਰਾਨ ਲਾਭਪਾਤਰੀਆਂ ਨੂੰ ਸੰਸਥਾ ਵਿੱਚ ਰਹਿਣ ਲਈ ਲੋੜੀਂਦੇ ਸਮੇਂ ਨੂੰ ਘਟਾਉਣ ਵਿੱਚ ਮਦਦ ਮਿਲ ਸਕੇ। ਕਿਸੇ ਮੈਡੀਕਲ ਸਹੂਲਤ ਵਿੱਚ 24 ਘੰਟੇ ਤੋਂ 60 ਦਿਨਾਂ ਤੱਕ ਘੱਟੋ-ਘੱਟ 1-ਦਿਨ ਦਾ ਮੈਡੀ-ਕੈਲ ਬਿਤਾਉਣਾ ਵਿਅਕਤੀ ਨੂੰ ਰਾਜ ਦੁਆਰਾ ਫੰਡ ਪ੍ਰਾਪਤ CCT ਲਈ ਯੋਗ ਬਣਾਉਂਦਾ ਹੈ।​​  

ਕੈਲੀਫੋਰਨੀਆ ਵੈਲਫੇਅਰ ਐਂਡ ਇੰਸਟੀਚਿਊਸ਼ਨ ਕੋਡ (WIC) ਸੈਕਸ਼ਨ 14196.2(c) ਦੇ ਆਧਾਰ 'ਤੇ, ਜਨਵਰੀ 1, 2026 ਤੋਂ ਸਿਹਤ ਸੰਭਾਲ ਸੇਵਾਵਾਂ ਵਿਭਾਗ (DHCS) ਉਨ੍ਹਾਂ ਮੈਡੀ-ਕੈਲ ਲਾਭਪਾਤਰੀਆਂ ਨੂੰ ਦਾਖਲ ਕਰਨਾ ਬੰਦ ਕਰ ਦੇਵੇਗਾ ਜੋ ਪਹਿਲਾਂ ਰਾਜ-ਫੰਡ ਪ੍ਰਾਪਤ CCT-ਵਰਗੇ ਪ੍ਰੋਗਰਾਮ ਲਈ ਯੋਗ ਸਨ। ਇਸ ਤੋਂ ਇਲਾਵਾ, ਜਨਵਰੀ 1, 2027 ਤੋਂ, DHCS ਰਾਜ-ਫੰਡ ਪ੍ਰਾਪਤ CCT-ਵਰਗੇ ਪ੍ਰੋਗਰਾਮ ਲਈ ਸੇਵਾਵਾਂ ਪ੍ਰਦਾਨ ਕਰਨਾ ਬੰਦ ਕਰ ਦੇਵੇਗਾ।​​  

Given this policy change, the last date an LO should enroll Medi-Cal beneficiaries under the State-Funded CCT-Like Program requirements is December 31, 2025.

Once the inquiry is reviewed and it is identified that a CCT LO is needed in a particular geographic area, the DHCS team will inform you of the next steps. All application questions can be sent to California.CommunityTransitions@dhcs.ca.gov.​​

Community Supports are services that Medi-Cal managed care plans (MCPs) utilize to address Medi-Cal members ‘ health-related social needs, help them live healthier, and avoid higher, costlier levels of care.  These include:​​ 

• ਨਰਸਿੰਗ ਸਹੂਲਤ (NF) ਸਹਾਇਤਾ ਪ੍ਰਾਪਤ ਰਹਿਣ ਦੀਆਂ ਸਹੂਲਤਾਂ ਦਾ ਪਰਿਵਰਤਨ/ਡਾਇਵਰਸ਼ਨ​​ 
• ਕਮਿਊਨਿਟੀ ਟ੍ਰਾਂਜਿਸ਼ਨ ਸੇਵਾਵਾਂ / ਨਰਸਿੰਗ ਸਹੂਲਤ ਘਰ ਵਿੱਚ ਤਬਦੀਲੀ​​ 
• ਕਈ ਤਰ੍ਹਾਂ ਦੀਆਂ ਹੋਰ ਕਮਿਊਨਿਟੀ-ਅਧਾਰਤ ਸੇਵਾਵਾਂ​​

CCT’s Relationship to Community Transition Services/Nursing Facility Transition to a Home:​​ 

• ਇੱਕ ਮੈਂਬਰ CCT ਪ੍ਰੋਗਰਾਮ ਅਤੇ ਕਮਿਊਨਿਟੀ ਟ੍ਰਾਂਜਿਸ਼ਨ ਕਮਿਊਨਿਟੀ ਸਪੋਰਟ ਦੋਵਾਂ ਲਈ ਯੋਗ ਹੋ ਸਕਦਾ ਹੈ ਪਰ ਇੱਕੋ ਸਮੇਂ ਦੋਵੇਂ ਪ੍ਰਾਪਤ ਨਹੀਂ ਕਰ ਸਕਦਾ।​​
