The following is the current managed care contract boilerplate for all models of managed care beginning CY 2024, including County Organized Health Systems, Geographic Managed Care, Regional, Single Plan, and Two-Plan models. *Please note: All Managed Care Plan contracts have been formally amended to extend the term through December 31, 2026.

This webpage and its linked documents provide information of public interest regarding the Department of Health Care Services’ (DHCS) executed amendments to Medi-Cal Managed Care Boilerplate Contracts (Amendments).
