Flexible Housing Subsidy Pools and Hubs

Flexible Housing Subisdy Pool (“Flex Pools”) are a mechanism to organize and administer funding from variious housing support programs and financing mechanisms including Transitional Rent, Community Support, Behavioral Health Services Act (BHSA), and other county, state, and federal housing programs to efficiently and effectively connect Medi-Cal Members to housing. Hubs contracted by MCPs to provide support to Community Supports providers, particularly those providing Recuperative Care and Short-Term Post-Hospitalization Housing, are well-positioned to administer Flex Pools. Flex Pools and Community Care Hubs

Flex Pool

  • Act as a fiscal intermediary between funders and landlords
  • Identify, secure, and support a portfolio of units for participants
  • Coordinate (and may contract with) housing community supports providers

Community Care Hub

  • Act as a fiscal intermediary between funders and providers
  • Contract or business associate agreement (BAA) with housing community supports providers
  • Coordinate funding streams
  • Contract (or BAA) with MCP
  • Provide substantial core administrative services
