There are new requirements for posting documents on a state government website. The Department of Health Care Services is reviewing all webpages and documents. As a result, some of the links or webpages are removed or temporarily unavailable. Please bear with us during this process.

When making a request for an ISCD document or webpage, please email ISCDWebpageInquiries@dhcs.ca.gov and include the Uniform Resource Locator (URL) of the requested document or webpage.
